POLAR BEAR LEAGUE TEAMS / TO-DO LIST

For all teams playing in the Polar Bear League - a Team package must be submitted to the OSU office at least two (2)weeks prior to your team’s first game, and must include all the items, as follows:

¨ Complete Team List (Player’s names)

¨ Ontario Soccer Aims Roster - Teams registering their players with their club MUST provide us with 
        an official Ontario Soccer Team roster as proof of players registration.

¨ Cheque (payable to OSU Soccer) for any outstanding fees:

**Please note, for any team that does not get their package to us on time,   
     we cannot guarantee that you will be eligible to play your first scheduled
      game.
